In others, filing any responsive pleading automatically constitutes an appearance. Self-Representation Designation
Indicate you are appearing 'Pro Se' (for yourself, without an attorney). Some states have a checkbox or require you to write 'Pro Se' or 'Self-Represented Litigant' on the form.
Certificate of Service
Certify that you served (mailed or delivered) a copy to the plaintiff's attorney. Include the attorney's name, address, and date of service. Sign and date.
